Do you know who created the initial classification system for adverse events in healthcare? Additionally, is it possible to rectify well-known errors? Contrary to popular belief, Ernest Amory Codman, not Lucian Leape et al., was the first to develop the adverse event system. The classification created by Leape et al. is known in the literature as Harvard Medical Practice Study I and II. And these studies have a template that emphasizes medication errors based on triggers.
In 1917 (1918 in some sources), Codman, a surgeon, examined this issue in detail (pages 98-105) in a book titled “A study in hospital efficiency: as demonstrated by the case report of the first five years of a private hospital.”
